At Doctors Lake Family Dental, we strive to provide you with the best patient care and make every effort to make this experience as comfortable and financially convenient as possible. We accept and file all PPO dental insurances and will be happy to do a complimentary benefits check. Our practice is considered in-network and participating providers with the insurance companies listed below:
- Cigna Dental PPO<
- Aetna (under the Dental Health Alliance Plan)
- Assurant (under the Dental Health Alliance Plan)
- United Concordia (under the Dental Health Alliance Plan)
- Sun Life (under the Dental Health Alliance Plan)
- Delta Dental Premier
Financial Policy
Payment is expected at the time of service. We accept cash, check, American Express, Discover, Mastercard, Visa and CareCredit. A discount of 5% for cash/check is available upon request to patients who pay in full and do not have insurance.
Reservation Policy
We reserve the right to charge a $50.00 per hour fee to your account for reservations that are missed or changed with a notice less than 2 business days.
If you do not confirm your reservation two business days prior to your visit, we reserve the right to cancel this reservation. A cancellation fee may apply.
If you arrive to your reservation fifteen minutes late or more, your visit may need to be rescheduled and a cancellation fee may apply.
